Your Initial Appointment
Your first appointment is confirmed with payment.
You will receive an txt message confirmation within (1) business day and a receipt upon request. (check whether a receipt can be downloaded upon payment)
Your Subsequent Appointments
A payment to confirm your appointment is required (1) business day prior to your appointment.
You will receive an confirmation SMS upon payment prior to your appointment.
Please note that your payment details will be securely stored with Stripe and a confirmation payment will be processed (1) business day before your appointment, unless you have contacted admin to reschedule.
What if I cancel my appointment?
I understand that there may be reasons why you may need to change your appointment time.
I just ask that you understand that I have set aside an hour for you and that you give me at least (1) business days’ notice if you need to reschedule or cancel your appointment.
If you do not attend your appointment and you do not contact admin (1) business day prior to your appointment to reschedule or do not have a medical certificate, MCCS reserves the right to charge the full appointment fee.
GP Referrals
I understand that I am unable to use a Mental Health Care Plan with a counsellor and I can only use the Mental Health Care Plan with a psychologist.
COVID-19
I consent that in the event that COVID-19 is suspected or confirmed in our offices, community and surrounds that Mind Centered Counselling Services, by law, must alert the Department of Health and include my name and telephone number.
I understand that my appointment may be changed to a Telehealth (zoom) appointment during a lock down or restriction period or if required by vaccination policy.
I understand and agree to wear a mask if directed by State or Federal Government policy.
I understand and agree to check into the office with the Safe WA App.
